Everyone's second Chess Book
NM Dan Heisman
He has a list of the 10 most common mistakes for those in the 1200-1600 range
** Not think about the opponent's moves...and being overly focussed on one's own attack
** Being in awe of the opponent
** Attacking before all the pieces are in play
** Falling for an opening trap
** Moving too fast. (He actually recommends sitting on your hand to resist this reaction)
** Not being good at "counting" (not sure if this was a mistake)

Counting is the most basic tactic. It's usually it's easy to count how many pieces are attacking a piece and how many are defending it . Beginners often count wrong when there a many piece attacking and defending. Usual mistake is not seeing that one of the defenders is pinned and thus not actually protecting anything.
I have read many good reviews about this book, but I think it is out of print at the moment. Was the book helpful to you? There is a second print coming soon and hopefully it is available before long..

I would also add not playing enough - if a person plays it/studys it for 3-4 hours a day within a year he will see a huge improvement.

The book is written by a very experienced chess teacher and gives a lot of examples for beginners to find their way around the chessboard
